Requiring Medical Treatment

Step 1: Notify your Leader of the incident

• If this is an emergency, call 911 immediately.

Step 2: Call 844-891-6022 to talk to NurseAdvantage

• Choose Option 7 when prompted

• Choose Team Schierl Companies as your employer

Step 3: A nurse will ask for information and guide the Injured Associate to the care they need.

Step 4: If medical treatment is necessary, the nurse will help over the phone or tell you where to go for treatment.

Step 5: Complete the following forms:

• Associate Worker’s Compensation Report

• Incident / Accident Report

Forward both reports to WilliamS@teamschierl.com and ChrisD@teamschierl.com

Forward the Return To Work release form (the medical provider will supply this form) and other information (hospital name, address, phone, doctor’s name) to ChrisD@teamschierl.com
